Our goal is to help older Veterans lead healthier and more satisfying lives.
To Join the Geriatric Clinic
Veterans can refer themselves or be referred by their primary care physician to the clinic.
Type of Services we offer:
- Geriatric Primary Care -we are the sole Primary care team for the Veteran
- Geriatric Evaluation and Management - this is a consulting service where the Veteran keeps their primary care physician and Veteran is seen by us for a few visits till they are stabilized
- Outpatient Palliative Care - this is a consulting service that Veterans are seen only once unless they have a real need to be seen again.
