If you were not completely satisfied with your experience with us, please contact Customer Service at (909) 583-6133 so that we have an opportunity to make immediate corrections. 

If your Patient Safety concerns are not addressed through any of the above, please contact Patient Safety Officer at (909) 825-7084 extension 2950 or leave a message on the Patient Safety Hotline extension 2505.  Patient Safety Hotline is available 24 hours/7 days a week.

For concerns with quality of care contact Loma Linda Customer Service at (909) 825-7084 extension 6133.

Concerns that involve decisions about the care of an individual patient/family can be addressed with an ethics consult or by calling the Ethics Hotline at (909) 583-6132.  If you call the hotline, we encourage you to leave your name so we can provide follow-up information.

You have the right to present concerns pertaining to patient care and safety at all VISN 22 Health Care System locations including Outpatient Clinics.  These concerns should be directed to the Manager of the Service or Department that is providing care, the patient advocate or the facility director.  If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor.

The Joint Commission

If you have a safety or quality of care concern which has been unresolved through hospital management, employees and patients have the right to contact The Joint Commission by dialing 1-800-994-6610 or email patientsafetyreport@jointcommission.org. Disciplinary action may not be taken against any employee or patient who reports a safety or quality concern to The Joint Commission.
