The clinic operates just like a local doctor's office where patients are seen by scheduled appointment.  The clinic is not an emergency center.  To enroll call the clinic to schedule an interview and begin your care.

Driving Directions

From the I-15, take Magnolia Ave. Exit (Right) on Magnolia Ave. for .7 of a mile.  Clinic is on the left side.  Move into the 1st left turn lane, passing the Clinic and make a "U" turn to come back to parking lot.
