Registration and Orientation

Trainee Registration and Training

Instructions for Trainees:

  • If you are paid by the Department of Veterans Affairs, please contact Human Resources Management Service (909) 825-7084 ext. 2872; these may not be the forms you are required to complete.
  • Trainees and students who are not paid by the VA and who will be here for more than 180-days must complete items 1, 2, 3, 4, 5, 6 and 7 (this includes, but is not limited to medical, nursing, pharmacy, occupational therapy and physical therapy students and psychology interns).
  • Trainees here for less than 180-days complete items 3, 4, 5, 6 and 7.
  • Housestaff (physician interns, residents and fellows) complete ONLY items 3, 4, 5, 6, 7 and 8.

Note: Make sure you have a printer available.  Some of these forms can not be saved.


1. Complete, print and sign  Questionnaire for Non-Sensitive Positions SF-85  or complete online using the U.S. Office Personnel of Management e-Qip system: http://www.opm.gov/e-qip/.

2. Provide a Resume, Curriculum Vitae or completed OF-612 (Residents are exempt from this requirement)

3. Complete and print VA Clinical Trainee Registration Form - VA Form 10-0410

4. Complete, print and sign as appointee  OF-306

5. Log into the Employee Education System External On-line Learning (LIBRIX) Website: https://www.ees-learning.net Click on the "First Time Users" button to register.

Once in the system:

  • Click on Available Courses on the left
  • Click on the "Explore" Tab in the upper right corner
  • Scroll Down
  • Click on the "Security" folder
  • You will need to click on "Sign Me Up" for the two courses
    1. General Employee Privacy Awareness
    2. VA Cyber Security Awareness
  • Submit your Certificate of Completion for each course

6. Read on-line, print and sign the last page of the VA National Rules of Behavior (NEW)

7. Read LLVAHCS Orientation Material

8. Read Understanding Resident Supervision

9. Read A Brief Wrap on Ethics

10. Read Guidelines for Interactions between Clinical Faculty and the Pharmaceutical Industry an Health Industry Practices that Create Conflicts of Interest

11. Complete, print and sign Attestation

Please bring completed forms and training certificates with you to Orientation.  Additionally you will need to bring:

  • Government-issued photo identification card (I.e. drivers license, military identification, passport)
  • Permanent Resident Card (if applicable)
  • Vehicle Registration and proof of insurance (if applying for VA parking permit)
